These three Honeywell fan controls were on Grandpa's shelf anf do show shelf wear. One looks new but has an elctrical fitting attached so it may have been installed at one time. That one is a model L4064B1469 and has a 5.25 inch insert. The other two have not been installed. One is a L4069A 1060 2, and it has a 7.25 insert. The final control is a L4064B 2228 5 and has a 5.25 inch insert.
This came from Grandpa's estate along with many other HVAC parts and some tools.
